Hole-in-One Donation

If a hole-in-one is made on the selected Rally for the Cure^® par 3 hole at your event, $2,500 will be donated to Susan G. Komen for the Cure in the name of the golfer, and your event will be credited.

The golfer will be asked to fill out the verification form which is attached. The winner must have 3 witnesses sign the form to qualify. This form can be returned with your entry fees and your subscription list.

Limited offer, Golf For Women's maximum donation is $25,000. Offer valid only if verified by witnesses.
